29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Production Area- On October 20, 2017, the employer failed to ensure that all areas where hazards created by pinch points at the Hold Down Area were guarded on Inserter Machine #2 to prevent injury to employees during the operation of the machine.      b) Production Area- On October 20, 2017, the  employer failed to ensure that all areas where hazards created by pinch points at the Hold Down Area and the point of operation on the end of the inserting line under the counter were guarded on Inserter Machine #4 to prevent injury to employees during the operation of the machine.      c) Production Area- On October 20, 2017, the  employer failed to ensure that all areas where hazards created by pinch points at the Turnover and the point of operation on the end of the inserting line under the counter were guarded on Inserter Machine #7 to prevent injury to employees during the operation of the machine.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed:    a) Production Area- On October 20, 2017, the employer failed to ensure that chains and sprockets within seven or less feet from the ground or working platform on the Inserting Machines #2, #4 and #7, were provided with a guard on all exposed sides to enclose the hazard posed by the moving equipment.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Production Area- On October 20, 2017, the employer failed to ensure that a gang box used to supply power to two flexible cords from one flexible cord near the Plow Fold Line was used in accordance with its listing and labeling.  The gang box was not intended to be used as a junction box where not affixed to a fixed surface.     b) Production Area- On October 20, 2017, the employer failed to ensure that electrical equipment, such as the multiple outlet relocatable power taps in the Production area, were being used only to power audiovisual equipment as permitted under its listing and labeling.  The multiple outlet relocatable power taps were used to power the Inserter Machine #4, the FCA Binding Line and other extension cords.     c) Production Area- On October 20, 2017, the employer failed to ensure that temporary wiring extension cords were not used in conjunction with other temporary wiring extension cords or relocatable power tap devices on Inserter Machine #2, Inserter Machine #4 and the FCA Binding Line.  These cords powered industrial equipment and relocatable power taps.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
