The gang box was not intended to be used as a junction box where not affixed to a fixed surface.     b) Production Area- On October 20, 2017, the employer failed to ensure that electrical equipment, such as the multiple outlet relocatable power taps in the Production area, were being used only to power audiovisual equipment as permitted under its listing and labeling.  The multiple outlet relocatable power taps were used to power the Inserter Machine #4, the FCA Binding Line and other extension cords.     c) Production Area- On October 20, 2017, the employer failed to ensure that temporary wiring extension cords were not used in conjunction with other temporary wiring extension cords or relocatable power tap devices on Inserter Machine #2, Inserter Machine #4 and the FCA Binding Line.  These cords powered industrial equipment and relocatable power taps.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
