29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. North side, fronting Fujita Road: Eye protection was not used when two employees used claw hammers to drive nails into lumber. Employees were exposed to eye and struck-by hazards.

Employees were exposed to electrical and fire hazards.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: East Side, Facing Chamorrita Way: A circular saw used to cut lumber for formwork was missing the ground pin. Employee was exposed to electrical hazards. Leading Tech Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.404(f)(6), which was contained in OSHA inspection number 1056800, citation number 2, item number 2 and was affirmed as a final order on October 19, 2015, with respect to a workplace located at Pia Marine Hotel, 193 Tumon Lane, Tamuning, GU 96913.
