The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:     On or about October 9, 2017, the employer did not ensure an employee implemented energy control procedures during the service and repair of a valve and piping of a computer controlled closed line tank cleaning system, known as the CIP skid #1, by isolating all energy sources [per the 1910.147(d)(3) requirements], affixing a lockout device to each energy isolating device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to starting work [(d)(6)].
