Main Street, Leeds, MA 01053, on 10/25/2017 and at times earlier the exposure control plan was dated 3/4/2016 and had not been updated or modified to include tasks and procedures for housekeeping and maintenance staff with occupational exposure to bloodborne pathogens. The Veteran's Health Administration was previously cited for a repeated violation of this occupational safety and health standard or its equivalent standard (1910.1030(c)(1)(iv)), which was contained in OSHA inspection number 950641, citation number 1, item number 1 and was affirmed as a final order on 1/7/2014, with respect to a workplace located at 1201 North Muldoon Road, Anchoraqe, AK 99504.
