29 CFR 1904.29(b)(1): 29 CFR 1910.1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation: a) On or about 10/25/2017: The employer failed to fill out the OSHA 300 2017 log with the detail required for case # 1 and #2: - Column 'C ' Job title was not filled in for each injured employee. - Column 'F' parts of the body affected or object that directly injured the employees was not entered for each of the employees. - Columns 'G - J' was not checked as required. - Column 'K' days away from work was not entered on the 300 In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
