One authorized employee provided his own lock for servicing/maintenance of equipment, but other employees/managers did not use locking hardware.  Employees perform maintenance and/or repair on equipment with multiple sources of energy including, but not limited to  the Goodyear Unicircle Tire Builder and the SMC Curing Chamber. This violation last occurred on or about January 15, 2018.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    The employer failed to ensure that each operator completed the forklift training and operator evaluation required by the standard before operating a forklift in the workplace. Employees operate a propane forklift to move objects such as tires in the facility.     The violation of failing to properly train forklift operators was last observed on October 24, 2017. The employer provided the required industrial truck training on October 27 - 28, 2017.     The certified evaluation of each driver's job-site required operating skills had not been completed by the employer. This violation was last observed on January 26, 2018.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    Employer is failing to ensure that flexible electrical cords were provided proper strain relief in the following instances:    A) The flexible electrical cord entering the Dayton motor of the Cahill Precure Builder was pulled loose from its strain relief.    B) The flexible electrical cord entering the panel box of the Cahill Precure Builder was pulled loose from its strain relief.    C) The flexible electrical cord of the plug for the Cahill Precure Builder was pulled loose from its strain relief.    D) The flexible electrical cord entering the heater portion of the CP rope rubber gun at the rope rubber fill station was pulled loose from its strain relief.    E)  The flexible electrical cord entering the 3-pedal foot control for the tire clamps machine was pulled loose from its strain relief.    These hazards were last observed on or about January 26, 2018.
