29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) New residential construction on Lot #14, Second floor: On or about 10/25/2017, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns at 120 PSI, in that protective eyewear was not provided to employees operating these tools while engaged in framing activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) New residential construction located on Lot #14, Second floor: On or about 10/25/2017, and at times prior, the employer exposed employees to fall hazards up to 15-feet, 8-inches from the lower level, in that a fall protection system was not provided to employees engaged in framing activities.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  (a) New residential construction located on Lot #14, First floor: On or about 10/25/2017, and at times prior, the employer failed to inspect the ladders used on site to access the upper levels, in that the side rails were bent in multiple areas and many rungs were damaged as this ladder was used to access the second level to perform framing activities.
