29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: a) At the facility, on or about November 2, 2017 - Elements of a written program were not implemented to ensure that any employee using a respirator voluntarily was medically able to use that respirator. Employees voluntarily wear an ArcOne Clean Air Flow during welding operations. The Woodward Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(2)(ii), which was contained in OSHA inspection 949692, citation number 1, item number 1a and was affirmed as a final order on 01/06/2014, with respect to a workplace located at 128 Myoma Rd, Mars, PA 16046.

29 CFR 1910.134(e)(6): 29 CFR 1910.134(e)(6): In determining the employee's ability to use a respirator, the employer shall obtain a written recommendation regarding the employee's ability to use the respirator from the PLHCP (physician or other licensed health care professional): a) At the facility, on or about November 2, 2017 - Employees, who voluntarily wear a respirator, were not provided with medical evaluation prior to donning a respirator. Employees voluntarily wear an ArcOne Clean Air Flow respirator during welding operations. The Woodward Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(e)(6), which was contained in OSHA inspection 949692, citation number 1, item number 1b and was affirmed as a final order on 01/06/2014, with respect to a workplace located at 128 Myoma Rd, Mars, PA 16046.
