29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5): Employees in the warehouse and employees using a Toyota forklift (Model 8FBE18U SN-13392) were exposed to struck-by hazards in that operator training was not completed to minimize the hazards associated with moving product with the forklift.

29 CFR 1910.253(b)(4)(i): Oxygen was being stored together in lieu of being separated by a distance of 20 ft. or by a non-combustible partition having a fire resistance rating of at least one hour. Employees in the Main maintenance shop were exposed to fire and explosion hazards in that an oxygen cylinder was stored with an acetylene cylinder.
