29 CFR 1910.28(b)(3)(iv): The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. On the third floor of the facility, next to the M63 machine, employees were exposed to fall hazards of approximately 8.5 feet. The self-closing mechanism of the gate guarding the fixed ladderway floor hole was not functional, allowing the gate to remain open approximately 11 inches.

29 CFR 1910.272(j)(2)(ii):The employer did not immediately remove any fugitive grain dust accumulations whenever it exceeded 1/8 inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators: At the first and second floors of the facility, employees engaged in sunflower seed production activities were exposed to explosion hazards in that the employer had not removed fugitive grain dust in excess of 1/2 inch that had accumulated on and around the floors and wall ledges, which are priority housekeeping areas. The elevator was in operation at the time of the inspection.

The horizontal shaft projected approximately 1 1/4 inch. 2) M36A Machine horizontal shaft diameter was approximately 1 1/4 inch. The horizontal shaft projected approximately 2 inches. 3) M17 Machine horizontal shaft diameter was approximately 1 1/4 inch. The horizontal shaft projected approximately 1 inch. 4) M18 Machine horizontal shaft diameter was approximately 1 1/4 inch. The horizontal shaft projected approximately 1 inch. Red River Commodities, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.219(c)(4)(i), which was contained in OSHA inspection number 1489855, citation number 1, item number 1-b and was affirmed as a final order on 03-09-2021, with respect to a workplace located at 340 40th Street NW.
