29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   Jason Disher worksite located at 21 Knollwood, Perrysburg, Ohio: On or about October 26, 2017, the employer did not ensure two employees were protected from head injuries while working below employees tearing off old shingles on a two story residential structure. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Jason Disher worksite located at 21 Knollroad, Perrysburg, Ohio: On or about October 26, 2017, the employer did not ensure the side rails of the extension ladders being used on the front side of the two story residential structure were extended at least three feet above the landing surface. One employee was exposed to falls while using the ladder to access the roof.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    Jason Disher worksite located at 21 Knollwood, Perrysburg, Ohio: On or about October 26, 2017, the employer did not ensure employees were protected from falls while tearing off shingles and making roof repairs on the two story residential structure. The employees were exposed to a fall distance of approximately 18 feet from a steep slope roof.   Jason Disher was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1182345, citation number 1, item number 2 and was affirmed as a final order on January 6, 2017, with respect to a workplace located at 2105 Woods Edge, Perrysburg, Ohio.
