A) Jason Disher on or about March 30, 2018, at the address of 101 Wellington Place, Perrysburg, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.   Jason Disher was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(1), which was contained in OSHA inspection number 1273301, citation number 1, item number 1A, and was affirmed as a final order on December 20, 2017, with respect to a workplace located at 21 Knollwood, Perrysburg, OH.

A) Jason Disher on March 30, 2018 at the address of 101 Wellington Place, Perrysburg, OH: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, personal fall arrest systems, guardrails, or safety nets, while applying new roofing materials to a residential structure during roofing operations.   Jason Disher was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(a)(1), which was contained in OSHA inspection number 1273301, citation number 1, item number 1B, and was affirmed as a final order on December 20, 2017, with respect to a workplace located at 21 Knollwood, Perrysburg, OH.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  A)    Jason Disher on or about March 30, 2018, at the address of 101 Wellington Place, Perrysburg, OH: The employer did not ensure employees were wearing appropriate eye protection such as, safety glasses while using a pneumatic nail gun to fasten asphalt shingles to the roof of residential structure. The employees using these tools in this condition were exposed to potential eye injuries from flying objects.  Jason Disher was previously cited for a violation of this Occupational Safety and Health Standard 1926.95(a), which was contained in OSHA inspection number 1182345, citation number 1, item number 1, and was affirmed as a final order on January 5, 2017, with respect to a workplace located at 2105 Woods Edge, Perrysburg, OH.

A) Jason Disher on or about March 30, 2018 at the address of 101 Wellington Place, Perrysburg, OH: The employer did not ensure employees were protected from falls while installing new roofing materials to a residential type structure.  The employees were exposed to a potential 20 foot fall to the level below while working from a 10/12 pitch roof.           Jason Disher was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1273301, citation number 2, item number 1, and was affirmed as a final order on December 20, 2017, with respect to a workplace located at 21 Knollwood, Perrysburg, OH.
