29 CFR 1926.20(b)(4): The employer did not ensure that the employee(s) required to operate equipment and machinery was qualified by training or experience. a) 6th and Linden Street, Allentown - On or about October 25, 2018 - An employee was working with an excavator to lift a section of road plate while utilizing an alloy steel chain sling, which did not properly attach to the section of road plate, but the employee had not received prior training on the safe use of lifting devices and slings.
