29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) On or about 11/8/17 at 55 Water Street, employees of SOHO-Dumbo, Inc. were exposed to physical hazards from chemicals because the employer did not provide and ensure the use of eye and face protection, where employees cleaned modular precipitator units of wood combustion byproducts in the industrial dishwasher using Tridex solution.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    (a) On or about 11/8/17 at 55 Water Street, employees of SOHO-Dumbo, Inc. were exposed to physical hazards from chemicals because the employer had not implemented a respiratory protection plan. Employees cleaned modular precipitator units of wood combustion byproducts in the industrial dishwasher using Tridex solution, for which they were required to wear half face, elastomeric respirators.
