29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a) Worksite:  On or about 10/31/2017, employees were not protected from the overhead hazard of pallets of roof shingles being hoisted (by a boom truck) onto the roof's surface.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:  (a) Worksite:  On or about 10/31/2017, the employer did not ensure that hinged roof anchors were installed as per the manufacturer's instructions, which are necessary to make them capable of supporting at least 5,000 pounds per employee. The hinged roof anchors were not fully secured to the roof surface.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Worksite:  On or about 10/31/2017, employees were not protected from falling while working on a roof up to approximately twenty-two feet above a lower level.  LDL Construction, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1248907, citation number 01, item number 001 and was affirmed as a final order on 9/29/2017, with respect to a workplace located at 77 Pillsbury Street, Concord NH, 03301.
