29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) Flexoseal 35 X 85 operator: On or about November 2, 2017 and at times prior to, an employee was exposed to noise dose at levels of 61.2% (equivalent to an 10 hour time weighted average TWA of 86.46 dBA) which is 1.53 times the action level of 40% (equivalent to an 10 hour TWA of 83.4 dBA) while working at the Flexoseal 35 X 85 machine and was not provided a baseline or annual audiogram. Capitol Container, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95 (g)(6), which was contained in OSHA inspection number 1215607, citation number 1 , item number 1 b and was affirmed as a final order on March 9, 2017, with respect to a workplace located at 2555 Container Drive, Montgomery, AL 36109.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k).
