29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) 674 Route 1&9, Elizabeth, NJ:  Employees who were operating tools such as but not limited to a masonry drill and chipper where not provided with and required to wear eye protection.  This hazard was observed on 10/11/17.

29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:  a) 647 Route 1&9, Elizabeth, NJ:  Employees were performing work from a pump jack scaffold that consisted of two scaffold planks which were not overlapped directly over a support.  This hazard was observed on 10/11/17.

29 CFR 1926.452(j)(2): Pump Jack Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  674 Route 1&9, Elizabeth, NJ:  a) Front of worksite:  The aluminum pump jack scaffold was not adequately braced and secured at the bottom of each pole to prevent displacement. 29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Front of 674 Route 1&9, Elizabeth, NJ:   Employees working from a pump jack scaffold were exposed to fall hazards of approximately 18 feet and were not protected by an adequate fall protection system.  This hazard was observed on 10/11/17.  b) Rear of 674 Route 1&9, Elizabeth, NJ:  Employees working from a pump jack scaffold were exposed to fall hazards of approximately 16 feet 11 inches and were not protected by an adequate fall protection system.  This hazard was observed on 12/7/17.
