On or about Thursday, August 10, 2018 at 85-38 Wicklow Place, Jamaica, NY 11432  The employer failed to certify abatement required for Citation 1, Items 1 (a), (b), and (c) which were issued on 03/05/2018.  Abatement certification for these items was due on 04/19/2018, 03/15/2018 & 04/19/2018 respectively.  ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)  (a) On or about October 25, at 85-38 Wicklow Place, Jamaica, NY 11432  The employer had not developed a written hazard communication program to cover employees who were using hazardous chemicals including Wind-lock Foam 2 Foam Professional Foam Adhesive (contains diisocyanates, dimethyl ether and propane and is extremely flammable and a respiratory sensitizer), Energex Enermix Dry stucco (contains Portland cement and crystalline silica and can cause skin and eye burns) and Energex Enerstyle coating (contains crystalline silica and is an eye and skin irritant).  ABATEMENT CERTIFICATION REQUIRED PURSUANT TO 29 CFR 1903.19
