29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  On or about November 2, 2017, at the above addressed worksite, an employee was exposed to a struck-by eye injury when using a pneumatic nail gun to perform roofing work while not wearing eye protection equipment.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   On or about November 2, 2017, at the above addressed worksite, employees were exposed to a fall hazard of approximately 10 feet 6 inches when gaining access to and from a roof by using a portable ladder that did not extend at least 3 feet past the roof edge and that was not secured to prevent accidental displacement.

Turnkey Construction Planners, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1164124, citation number 1, item number 1 and was affirmed as a final order on 09/12/2016, with respect to a workplace located at 11356 SW Patterson Street in Port Saint Lucie, Florida.
