29 CFR 1926.502(f)(2): Warning line systems did not consist of ropes, wires, or chains, and supporting stanchions erected in accordance with (f)(2)(i) through (f)(2)(v) of this section: a) Helena City Plaza, 1415 11th Avenue, Helena, MT: On October 31, 2017, and at times prior, the employer failed to properly flag the warning line at not more than 6 foot intervals. b) Helena City Plaza, 1415 11th Avenue, Helena, MT:: On October 31, 2017, and at times prior, the employer failed to ensure that the warning line did not sag below 34 inches. c) Helena City Plaza, 1415 11th Avenue, Helena, MT: On October 31, 2017, and at times prior, the employer failed to ensure that the tensile strength of the: rope, wire, or chain was rated at 500 pounds tensile strength.
