29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) Progressive Services, Inc. dba Progressive Roofing, 1100 W. 120th Ave., Westminster, CO 80234: On or before September 10, 2018, the exposing employer did not ensure that employees were protected from injury while installing waterproofing on a flat roof of a commercial building, in that the employer attempted to utilize a safety monitoring system on a section of the roof which was approximately 35 feet in width.  On September 10, 20118 an employee who was using a walking welder fell approximately 13 feet to a lower roof section.  The safety monitor was conducting work along with the other employees on that roof section.  This condition exposed employees to falls of approximately 13 feet.

The employer instituted a monitor only fall protection plan. This condition caused another employee to fall approximately 13 feet to the lower level roof.
