A device such as a panic bar that locks only from the outside is permitted on exit discharge doors:    On or about  November 3, 2017, employees in the Breeding Barn (Barn 5) were exposed to smoke inhalation and burn hazards when the exit doors could not be opened:    a) North Exit Door - secured by a wire attached between the handle and door jamb; and    b) South Exit Door - missing the handle and deadbolt knob needed to operate the closure mechanisms.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or about November 3, 2017, the employer did not provide a suitable face and body wash for quick drenching in the barns and employees were exposed to the following chemicals when making dilutions, cleaning and sanitizing (crates, mats, dividers, feeders, slats, louvers, fan blades, housing units, walls, floors, and the ceilings) with the following:    a) Bleach (stock 5.25-percent with a pH at 12.75 diluted 1:1 to pH at 12.68); and    b) Barnstorm (pH of 1 to 2).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    On or about November 3, 2017, employees were exposed to detergents and disinfectants and the employer did not develop, implement, and maintain a written hazard communication program with at least a description of how the criteria for the following requirements would be met by the company:    a) use of labels and other forms of warning,  b) access to safety data sheets,  c) provision of hazardous chemical information and training to employees,  d) maintenance of an updated list of hazardous chemicals present at the job site,   e) methods the employer will use to inform employees of the hazards of non-routine tasks and hazards associated with chemicals contained in unlabeled pipes in their work areas.  f) methods the employer will use to inform other employers on-site on their  precautionary measures to protect employees during normal operating conditions and in foreseeable emergencies, the labeling system used in the workplace and to provide other employers on-site access to safety data sheets.
