29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about November 3, 2017, at Lot 2, 433 Paden View Court, and Lot 7a, 2519 Paden Birch, Lawrenceville, Georgia:    a) An employee performing framing work was exposed to struck-by hazards from falling tools and materials of the employees working above him when he worked from the ground without using a hard hat.     b) Four employees performing framing work were exposed to struck-by hazards from trusses and the tools of the employees working above them when they worked beneath those employees without wearing hard hats.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about November 3, 2017, at Lot 2, 433 Paden View Court, and Lot 7a, 2519 Paden Birch, Lawrenceville, Georgia:    a) Two employees performing framing work were exposed to a fall hazard of approximately 25-feet and greater when they worked from the roof of Lot 2, 433 Paden View Court, without using fall protection.     b) Three employees performing framing work were exposed to a fall hazard of approximately 19.9-feet and greater when they worked from the top plate and unprotected edge of a house without using fall protection

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about November 3, 2017, at Lot 2, 433 Paden View Court, Lawrenceville, Georgia:    Five employees performing framing work were exposed to a fall hazard of approximately 13-feet when they used an aluminum extension ladder that had not been extended three feet above the second floor landing.
