29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. ESTABLISHMENT: The employees performing cleaning and sanitizing process of a 1500-gallon Solution tank were neither provided with nor wearing appropriate types of personal protective equipment (PPE), such as (but not limited to) protective face shield, heat resistant waterproof coat, pants or other equivalent protection over the rubber boots to protect severely burn hazards from heated and/or scalding hot water. On or about 11/03/2017, the scalding hot water splashed onto an employee who was performing a cleaning process of the above tank, and the employee sustained severe burns to left hand, leg and foot.
