29 CFR 1904.29(b)(3): Each recordable injury or illness were not recorded on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. a) Injury / Illness log (Form 300) for CY 2017 maintained by Old Dominion Freight Line, Inc. for their trucking terminal at 180 Tallmadge Rd in Edison NJ was not completely timely, in that known recordable work-related injuries occurring between 9/16/2017 and 10/26/2017 were missing from the Form 300 Log kept by the employer as of 11/06/2017. These recordable injuries include a 9/16 strained/sprained arm during material handling with 16 days of job restriction /transfer; a 10/09 struck-by injury resulting in broken foot and days away from work as well as job restriction / transfer; a 10/18 injury in which a dock worker was struck by a deck bar and sustained head lacerations with 4 days of job restriction / transfer; and a 10/26 forklift accident resulting in 2 days away from work. Condition(s) were noted on or about 11/06/2017.
