29 CFR 1910.179(n)(3)(xi): The warning signal was not sounded whenever the bridge was started or when the load or hook approached near or over personnel. On or about October 31, 2017 and at times prior there to, employees were exposed to being caught in the tag line and a subsequent fall when continual communication was not maintained while lifting a load at the wind turbine site.
