29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about November 2, 2017, the employer did not ensure procedures were utilized for the safe application, usage, and removal of potentially hazardous energy controls during the service and repair of a conveyor belt at the Press #2 coinc conveyer.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about November 2, 2017, the employer did not ensure employees implemented energy control procedures during the service and adjustment of the conveyer belt of the Press #2 coinc conveyer by, turning off and/or shutting down [per the 1910.147(d)(1) requirements], isolating all energy sources [(d)(3)], affixing a lockout device to each energy isolating device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to starting work [(d)(6)].
