On or about November 07, 2017, at the above referenced worksite, safety inspections were not made by persons capable of identifying and correcting those hazards which could result in employee injuries. As a result, corrective action was not taken with regard to ladder deficiencies, hazardous work zones, and deficiencies in personal protective equipment.
