29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  The employer failed to submit documentation of abatement within 10 days of the abatement date for Citation 1 Item 2, and Citation 1 Item 3.

29 CFR  1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about April  23, 2024, four employees were performing roofing activities at heights greater than six feet did not receive training on fall hazards leaving employees unable recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April  23, 2024, four employees were observed using an extension ladder to access the roof of a residential structure, the ladder being used did not have side rails that extend 3 feet past the landing surface.
