29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Facility, 112 Nardis Drive, Jefferson, PA: On or about October 24, 2017, an employee was operating a Dumore tool post grinder, series 57, that was mounted on a south bend-nordic 15 lathe and suffered an amputation when his finger got pulled into the ingoing nip point between the grinding wheel and the rotating lathe.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: a) Facility, 112 Nardis Drive, Jefferson, PA: On October 24, 2017, an employee was injured, admitted to the hospital and suffered an amputation and the employer did not report it to OSHA until October 30, 2017.
