On or about October 20, 2017, Warehouse: The employer did not ensure that each employee working on the racking system in the warehouse with unprotected sides greater than four feet above the lower level, were protected from falls to a lower level. An employee was severely injured when he fell from a rack on the third level of Mod 4, Level C, 17-feet to the concrete floor below.
