29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: Slapper Desk Area: On, or about, August 28, 2024, Crown #5, a Model RC5540-40 stand-up forklift was used to carry and empty a U-Line Model D-200-LD dumping hopper. The dumping hopper was not listed as an attachment on the identification plate and the calculated (estimated) reduced capacity of the forklift was reduced to approximately 16% of the original capacity of the forklift with the hopper in the dumping position.
