29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)  Denver Machine Shop, at 9762 Hanover Court E., Henderson, CO 80640:  On and before November 9, 2017, the employer did not develop, document, and utilize procedures for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  The employer operated machines with multiple energy sources such as, but not limited to, the Betts Horizontal Lathe, the Horizontal Boring Mill VWF, the Craven Lathe, the Craven Horizontal Lathe, the Archdale Drill Press, the Niles Horizontal Press, the American 48" Lathe, and the CNC Lathe.  The employer did not develop, document, and utilize the lockout tag out procedures associated with this equipment.  This condition exposed employees to amputation, caught in-between, struck-by, and electrical hazards.

The employer operated machines with multiple energy sources such as, but not limited to, the Betts Horizontal Lathe, the Horizontal Boring Mill VWF, the Craven Lathe, the Craven Horizontal Lathe, the Archdale Drill Press, the Niles Horizontal Press, the American 48" Lathe, and the CNC Lathe.
