29 CFR 1910.242(b): Compressed air, which was not reduced to less than 30 PSI, was used for cleaning purposes without effective chip guarding and personal protective equipment: On or about December 11, 2025, employees using pneumatic airlines when cleaning off equipment and themselves were exposed to hazards created from high pressured air. The employer failed to ensure that the airline(s) which was set at 105 PSI, was equipped with a safety nozzle designed to reduce pressure. Employees, and other workers in the immediate area, were potentially exposed to struck-by hazards from flying debris, hearing damage from loud noise and/or air embolisms.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. The employer did not report a work-related in-patient hospitalization to OSHA within 24 hours. On November 8, 2025, at approximately 11:40 AM an employee operating a Betts Vertical Boring Mill was injured when struck by the machine. The employee was transported to a hospital and admitted shortly after. The employer reported the incident to OSHA on November 25, 2025 at approximately 4:17 PM.
