29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Employees were exposed to amputation and electrical hazards. There was no written lockout tagout program. There are no machine specific lockout tagout procedures for equipment for equipment such as but not limited to, the Cincinatti Shear which is electric and pneumatic powered, and the Hydmech horizontal band saw which is electrically powered.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees were exposed to being struck by, and amputation hazards, while rolling material on the Pope three ring roller. There was no machine guarding provided to protect employees from ingoing nip points and rotating parts from the rollers.
