29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: a. On or about November 9, 2017, employees working in the sales office were exposed to unsanitary conditions from mold growth in the supply air vents that were contaminated with Cladosporium black mold growth that had not been cleaned. b. On or about November 9, 2017, employees working in the upper computer office and storage office spaces were exposed to unsanitary conditions from cock roaches, mold growth in the supply air vents that were contaminated with Cladosporium black mold growth, and accumulations of rodent feces on the desks and floors that had not been cleaned. c. On or about November 9, 2017, employees who used the back bathroom were exposed to unsanitary conditions from mold growth in the air vent that was contaminated with Cladosporium black mold growth that had not been cleaned.

On or about November 9, 2017, in the sales, computer and storage office spaces, the vermin extermination program was ineffective in that the warehouse offices continued to be infested with rodents, exposing employees to close contact with rodents and accumulations of rodent feces.
