29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. Facility: On or about October 24, 2017, employees working in the facility were overexposed to carbon monoxide that resulted with one(1) employee hospitalized with carbon monoxide overexposure and the incident was not reported to OSHA within twenty-four (24) hours.
