29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about November 13, 2017, and times prior thereto, at this site, authorized employees did not receive training to acquire the knowledge and skills required for energy isolation such as the tortilla machine. The operators are required to clean the machine every night. The operators are exposed to the hazard of being caught-in the machine from an unexpected start-up.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about November 13, 2017, and times prior thereto, the operators when cleaning the tortilla machines were not locking the energy sources out. The employer failed to ensure that locking devices were installed on the tortilla machines when cleaning the machines at the end of day. Employee(s) were exposed to the hazard of being caught in the rotating belt, rotating shelves, the press, and heat.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about November 7, 2017, the tortilla machine, identified as X-Press tortilla machine was not adequately guarded. 2. The end of the conveyor presented a catch point for clothing or fingers to be dragged into the conveyor belt. 3. There was no guard to prevent employees from getting their hands or fingers smashed from the press. b) On or about November 13, 2017, the replacement tortilla machine the tortilla machine was not adequately guarded. 2.
