29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) Facility, 1150 Second Avenue, New Kensington, PA: On or about November 14, 2017 authorized employees who perform servicing and maintenance on equipment had not been trained in the recognition of applicable energy sources or the methods and means necessary for energy isolation. A maintenance employee was injured when a pneumatic lift table being serviced lost air and fell. The employee had not been instructed on the methods or means necessary for energy isolation and had not isolated the air or used blocks to control potential release of energy.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Facility, 1150 Second Avenue, New Kensington, PA: On or about November 14, 2017 the employer had not conducted a periodic inspection of the energy control procedure at least annually to ensure that the requirement of this standard was being followed. The employer, UniFirst Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i), which was contained in OSHA inspection number 796181, citation number 1, item number 2 and was affirmed as a final order on March 18, 2013, with respect to a workplace located at 205 Garfield Avenue, Stratford, CT 06615.
