29 CFR 1926.451(e)(9)(i): The employer did not provide safe means of access for each employee erecting or dismantling a scaffold. a) Employees were exposed to 100 ft fall hazards when accessing the tubular welded frame scaffold by climbing on the frames. Employees were altering the scaffold by passing up scaffold planks 15 ft up within the scaffold from 6fl to 7fl level and there was no safe means to access the work areas of the scaffold. --- A scaffold stairway was not provided as required by the scaffold design plan. 29 CFR 1926.451(f)(7): The employer did not ensure that scaffolds were erected, moved, dismantled, or altered only under the supervision and direction of a competent and qualified person. a) The employer failed to ensure that a competent person supervised and directed the alteration of the tubular welded frame scaffold. Employees were exposed to a 100ft fall hazard while moving the scaffold planking of a work platform by passing the scaffold planks up to the floor above manually while standing on the scaffold frame. Safe access between the levels of scaffolding was not provided. Location: 422 Fulton Street, Brooklyn, N.Y. 29 CFR 1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds. a) Employees were exposed to 100 ft fall hazard when working and accessing the tubular welded frame scaffold. Employees were using a single lanyard personnel fall arrest system when working and moving on the scaffold. The 6' lanyard did not provide continuous fall protection as the employees were working on the scaffold. Employees would have to disconnect to keep moving and working on the scaffold. --- Employer did not ensure that employees use fall protection during scaffold erecting activities; Location: 422 Fulton Street, Brooklyn, N.Y. 29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. a) The employer failed to have a competent person train the employees on the means and methods to alter the scaffold by moving the work platform planks from the 6th floor to the 7th floor of the core elevator shaft tubular welded frame scaffold. Employees were exposed to a 100 ft. fall hazard while moving the scaffold planks of the work platform by passing the planks up to the floor manually while standing on the scaffold frames. Safe access between the levels of the scaffold was not provided and the designed plans for the scaffold were not followed. Location: 422 Fulton Street, Brooklyn, N.Y.
