29 CFR 1910.178(l)(1)(i):     The employer had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (l) [Reference 29 CFR 1926.602(d) for construction]:      a)  325 Whippany Road, Building #2, Whippany, NJ and at the Corporate Offices at P.O. Box 1625, Delran, NJ.    The employer had not ensured that employees operating the Terex rough terrain forklift had received training specified by this standard exposing them to injuries from improper forklift operations.
