29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition: Employees are exposed to fire and explosion hazards. Employees working in the milling basement are exposed to fire and explosion hazards. More than four and a half inches of oat dust, a combustible dust, was allowed to accumulate on surfaces.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: Employees are exposed to respiratory hazards. Milling employees are allowed to use respirators and the employer did not provide the information contained in appendix D of the standard. Employees are exposed to respiratory irritants such as oat dust.
