On or about November 17, 2017, employees were exposed to a struck by hazard. The employees were installing masonry materials from a supported scaffold on a new residential home located at 18 Harvester, Copley, Ohio 44321. Employees were working on upper levels of the scaffold with tools and materials while other employees were working below them without protective helmets.

The employees were installing masonry materials from a supported scaffold on a new residential home located at 18 Harvester, Copley, Ohio 44321. The working level of the scaffold was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about November 17, 2017, employees were exposed to a fall of approximately 13 feet. The employees were installing masonry materials from a supported scaffold on a new residential home located at 18 Harvester, Copley, Ohio 44321. The scaffold did not have a safe access point; employees were using frame rails and cross braces to access working levels.

The employees were installing masonry materials from a supported scaffold on a new residential home located at 18 Harvester, Copley, Ohio 44321. The working level of the scaffold did not have a proper guard rail system in place and the employees were not using any other form of fall protection.
