29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph: On or about July 9, 2018, employees were exposed to a fall of approximately 26 feet while installing masonry materials on a new residential apartment building located on Heritage Woods Drive, Akron, Ohio 44333. The employees were working from a supported scaffold that was greater than the 4:1 height to base requirement and the employer did not ensure that it was tied into the building.

29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section: On or about July 9, 2018, employees were exposed to a struck by hazard while masonry materials were being installed on a new residential apartment building located on Heritage Woods Drive, Akron, Ohio 44333. The employees were working from a supported scaffold and the employer did not ensure that toe boards were installed to protect employees on the ground from falling tools and materials.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about July 9, 2018, employees were exposed to a fall of approximately 26 feet while installing masonry materials on a new residential apartment building located on Heritage Woods Drive, Akron, Ohio 44333. The employees were working from a supported scaffold and the employer did not ensure that a complete guard rail system was in place. ROR Masonry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.451(g)(1), which was contained in OSHA inspection number 1277871, citation number 1, item number 4 and was affirmed as a final order on 12/21/2017, with respect to a workplace located at 18 Harvester, Copley, Ohio 44321.
