29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:    On or about November 16, 2017, employees were exposed to fire hazards when exit signs were not visible in the main warehouse area because the bulk storage blocked the view of the exits and exit signs along the walls.  The following exits were not visible from within the main warehouse:  a.  exit at bulk storage  b.  the exit close to rack location E020    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident:    On or about November 16, 2017, an employee involved in an accident approximately two weeks prior had not received refresher training.  The employee had been disciplined but there was no plan to retrain the employee until the next plant wide training class.  Employees who strike the racks, drive too fast, and do not pay attention to their surroundings are not re-trained.      No abatement certification or documentation is required for this item.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts if they were exposed or from the enclosure front or opening if they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:  On or about November 16, 2017, employees were exposed to electrical hazards when the parking place for the scrubber when not in use was in front of electrical panels in the ambient room.
