29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Celestial Seasonings, Inc. at 4600 Sleepytime Dr, Boulder, CO 80301: On and before November 13, 2017, machine specific, step-by-step procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in product changes at a C50 tea bagging machine. The tea bagger was improperly machine guarded with interlocked panels, which could be bypassed to reactivate the machine at a reduced rate with a hand control. During product changes employees worked inside the C50 with hands near points of operation, while bypassing the interlocked machine guards and activating the machine. The employer did not implement lock out procedures to ensure that employees did not make incidental contact with activated points of operation. On November 9, 2018, an employee's hand was drawn into a blade in the C50 tea bagger, resulting in the amputation of two fingers, while realigning tea bag paper in the C50 during a product change. This condition exposed operators to amputation hazards.

29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: (a) Celestial Seasonings, Inc. at 4600 Sleepytime Dr, Boulder, CO 80301: On and before November 13, 2017, the employer did not establish clear and specific procedures and requirements for testing the C50 tea bagger to determine and verify the effectiveness of lockout devices. The tea bagger had a hand control capable of bypassing interlocked machine guards to activate the machine but the employer did not establish clear and specific procedures to determine and verify the effectiveness of lockout devices at the hand control. This condition exposed maintenance workers to amputation hazards.

This condition exposed employees to caught between hazards.
