Milroy Enterprises, Inc. dba Graffius Burial Vault - The employer did not develop a written respiratory protection program for employees who are required to wear 3M half-masked respirators while pouring concrete into various molds, on or about November 20, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
