29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: (a) Milroy Enterprises, Inc. dba Graffius Burial Vault - A floor hole approximately three feet by three feet by three feet located at the concrete mixer discharge was not protected by covers or a guardrail system on or about November 20, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) Milroy Enterprises, Inc. dba Graffius Burial Vault - The strain relief provided for the flexible cord pendent used to operate the 2 ton Coffing electric hoist was not properly installed to prevent pull from being directly transmitted to joints or terminal screws on or about November 20, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
