29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: On or about November 10, 2017, in the old crib area, the employer did not evaluate the respiratory hazards for employees exposed to hazards associated with chemicals such as Methyl n-Amyl Ketone, n-Butyl Acetate, p-Chlorobenzotrifluoride, Hexamethylene Diisocyanate, and Hexamethylene Diisocyanate Polymer, which are included in ArmorSeal Hi-Solids Polyurethane Floor Enamel used as top coat during floor refinishing. Employees were exposed to inhalation hazards that could result in health effects such as but not limited to respiratory tract irritation, asthma symptoms, headaches, drowsiness and nausea.
