On or about November 22, 2017, at the A&AT, LLC plant in La Porte, Texas, the employer failed to ensure employees followed energy control procedures to verify the Step 2 "C" Reactor isolation valve remained locked out/ tagged out and the isolation was validated prior to taking the blind off the piping connected to the reactor.
